Ascend Transition Services is a 501C3 organization dedicated to providing families with post- secondary transition support through various Life-Skill Adult Day Training Programs, nursing services, companion support, pre-vocational programs, employment support, and community based projects.


We strive to be a comprehensive, compelling, and dedicated service. While remaining compassionate to the various needs of our members.


We are a diverse group of professionals ready to support our community.

Our vision is to create an inclusive community in SWFL by teaching families, businesses, corporations, and organizations strategies that support adults with intellectual/developmental disabilities.


Our mission is to provide life-long care to children and adults with intellectual/developmental disabilities by creating inclusive spaces and opportunities within SWFL.

Our Founders

Our founders are dedicated advocates who believe in the power of inclusion and the strength that comes from embracing every individual's abilities. Both are dedicated to empowering the SWFL community. Bridget’s unwavering commitment to individuals with intellectual/developmental disabilities motivated her to exit the classroom setting and create a space where individuals can safely transition into the community and adulthood. Rick's background as a First Responder/Fire Fighter & Personal Trainer motivated him to co-found Ascend and create inclusive cultures within medical and emergency settings, as well as gyms. Both are dedicated to supporting the SWFL community.
